Category A:
- For family with household income less than RM3,000 monthly
- Covers husband/wife and children below age 21
- One card with credit limit RM500 per year will be given to a family
- Card limit: RM50 per treatment
- Type of illness/treatment: No limit
- For individual below age 21 with income less than RM1,500 monthly
- A card with credit limit RM200 per year will be given
- Card limit: RM50 per treatment
- Type of illness/treatment: No limit
T&C for application:
- He/She must be a Malaysian
- Born in Selangor or stayed in Selangor for more than 10 years
- Download and complete the forms and submit to any ADUN offices nearby
